InternetArchiveBot needs to check a link a minimum of 3 times before conclusively declaring it dead, and that has not happened yet.
There is a minimum waiting period before the bot conclusively determines the link is dead, and that period has not passed yet.

To summarize, it seems to happen only with most image calls. It is not affecting template styles. And, for some reason, SOME image calls still work right.
Including screenshots for comparisons of where it happens and doesn't happen. The screenshots in Vector22 are Wikipedia, the oldschool Vector screenshots are MDWiki.
Before I get a quote for working on this, I want to make that the problem this is meant to solve – pulling data from a third-party service with consent – is actually the problem we want to solve, and not, say, putting the proxy in Wikimedia production.
My attempt at rewriting from scratch did not succeed in a timely manner, and I've come to the conclusion Imagestack is a good enough base to build on. Since Imagestack otherwise works in loading the images and providing scroll through functionality (without the scrollbar), I am going to scope the work around two tasks:
One discovery I made is that Imagestack already lets you scroll through the images. You can also click and drag up and down on the images to achieve this effect. This of course is not obvious due to the lack of scrollbar. I am going to see if we can get a scrollbar implemented on Imagestack. May also rewrite it as a Lua module to loop through parameters. Also need to figure out how to get it to work on mobile.

Currently, MDWiki uses https://mdwiki.org/wiki/Template:Imagestack to organize each image in the CT scan as an image in a slideshow, and you click a link to iterate to the next slide.
Also of note, the current scan viewer doesn’t quite work on mobile; it just shows each picture sequentially
