Duplicate Driver License Check List
The requirements are:
Complete a Driver License or Identification Card Application form (DL 44). To obtain this form by mail, call DMV's automated phone service 24 hours a day, 7 days a week at 1-800-777-0133.
Include your parents' or guardians' signatures on the DL 44 if you are under 18 years old.
Make an Appointment(s) to visit a DMV office.
Pay the application fee.
Additional information regarding a duplicate driver license or identification (ID) card.
